Case Management
Our Family Support Workers (FSW) will provide comprehensive case management, home visits, and act as liaisons with other service providers as necessary to assist participants in accessing available services such as medical care, housing, food, education, financial assistance, employment, mental health services, and legal assistance. FSWs work with participants to develop personal goals and objectives, to obtain resources and to overcome obstacles. The goal of our case management is for the participant to be encouraged and empowered to accomplish goals in their lives so they become self-sufficient.

Referrals
Family Support Workers make referrals to specialized services and intensive case management.
